Well I did play in CQB this Saturday and I may or may not have accidentally left the timing plug out of one of my nades and pulled it off my vest, detonating the grenade instantly and killing my 3 nearby teammates.

Trust me.. we have had every possible "accident" with these things.. When they do anything unexpected.. it can typically be attributed to improper priming or deployment by the user.

Oh it didn't just pull out or something. I had just never put the plug in, in the first place when loading the grenade.

Apply some tactical electric tape through a Molle strap and secure the pin to the top or another Mollie strap there no dangle and one hand toss
